Common Table Health Alliance

Common Table Health Alliance is a regional health and healthcare improvement collaborative for the greater Memphis area.Common Table Health Alliance is made up of its Community Partners: organizations, coalitions or individuals dedicated to improving health and healthcare of people in the greater Memphis area.

C3 Partners

C3 Partners’ approach to healthcare has provided creative and powerful operational tools. Their current product and service suite is called Meaningful Use Monitor (MUM), which supports hospital and medical groups in their pursuit of ARRA HITECH incentive funds. Their newest product and service is called MACRA Monitor.

DHS: Department of Human Services

The Wisconsin Department of Health Services (DHS) is one of the largest and most diverse state agencies in Wisconsin. DHS is committed to protecting and promoting the health and safety of the people of Wisconsin, making sure everyone can live their best life. DHS oversees Medicaid, the single largest program in the state budget.

HCIF: Health Care Improvement Foundation

HCIF is an independent, nonprofit corporation leading healthcare initiatives to improve the care experiences of all patients across the five-county greater Philadelphia region. HCIF supports the vision that an engaged, collaborative community will benefit from a high-performing healthcare delivery system that measures safety, quality, and effectiveness.

Johns Hopkins Center for Civil Society Studies

The Center conducts research and educational programs that seek to improve current understanding, analyze emerging trends, and promote promising innovations in the ways that government, civil society, and business can collaborate to address social and environmental challenges.

MHQP: Massachusetts Health Quality Partners

Massachusetts Health Quality Partners is a broad-based coalition of physicians, hospitals, health plans, purchasers, patient and public representatives, academics, and government agencies working together to promote improvement in the quality of health care services in Massachusetts.

MNCM: Minnesota Community Measurement

MN Community Measurement is a collaborative effort including medical groups, clinics, physicians, hospitals, health plans, employers, consumer representatives and quality improvement organizations–all of whom support the notion that greater transparency & measurement in our health care system will lead to better health outcomes for the people of Minnesota.

PA SPREAD: Pennsylvania Spreading Primary Care Enhanced Delivery Infrastructure

In an effort to spread the Medical Home model and better support primary care practices in making these changes, the federal Agency for Healthcare Research and Quality (AHRQ) has chosen 4 states (PA, NC, NM, OK) to lay the groundwork for a Primary Care Extension Service. The work in Pennsylvania is called PA SPREAD.


ReadyList is a customizable, web-based application that helps hospitals and clinics improve patient safety, clinical staff satisfaction and overall efficiency. By collaborating with facilities to standardize enterprise workflow and process, ReadyList’s team configures a solution to each facility’s specific needs. ReadyList helps optimize key workflows like inpatient admission/discharge, infection prevention and operating room turnover.

WAPC: Wisconsin Association for Perinatal Care

WAPC is the largest perinatal care association in the United States, and remains a leader in the advancement and improvement of perinatal care. While the health care environment continues to change, the mission and purpose of WAPC remains stable–to be an association that addresses perinatal care issues.

WisPQC: Wisconsin Perinatal Quality Collaborative

The Wisconsin Perinatal Quality Collaborative (WisPQC) focuses on the perinatal period–the time prior to pregnancy and through the first year of the infant’s life. The Collaborative offers opportunities for stakeholders to engage in quality improvement that can improve perinatal outcomes.

WMS: Wisconsin Medical Society

The Wisconsin Medical Society is the largest association of medical doctors in the state with more than 12,000 members dedicated to their patients. With that in mind, WMS offers patients a unique source for reliable, physician-reviewed medical information. WMS has been a trusted source for health policy leadership since 1841.
